December 2018 Influenza vaccine walk in clinics:
Every Friday 9-11am & 2-4pm until supplies are exhausted

Everyone is at risk for seasonal influenza. The CDC (Center for Disease Control) recommends everyone six months and older get a flu vaccine.
Here at Hancock Public Health we have the standard dose vaccines (6+mon and older) and the High Dose Vaccines (65+) available.
We accept most major insurances, including Medicaid and Medicare Part B. 
We also accept check or cash (no credit or debit) for individuals without insurance. 
Standard dose is $40.00 
High Dose $60.00

You must have your I.D., Insurance Card, and be accompanied by a guardian if you are under the age of 18. If you are not biological parent further consents and paperwork may be necessary, please call our office for further information. 
It is important to have these with you for us to protect your privacy and assure accurate billing to your insurance company. 

Please contact the nursing department with any questions at 419-424-7441 or if these dates and times do not work with your schedule, we will do our best to accommodate and schedule during an alternate clinic time.
